Dr. Shruti Agarwal

Dr. Shruti Agarwal is a clinician practicing for last 20 years. Her area of interest is clinical endodontics. She focuses on painless endodontics which is her forte. She developed her skills through diligent learning for many years under many renowned clinicians. Special mention should be made about Dr. Barin Roy who is considered the Father of Cliical Dentistry of Bengal who has been one of the mentors of Dr. Shruti. Endodontics is a subject which requires sound diagnostic skills as well as a precise clinical hand which comes only through years of perseverance. Dr. Shruti has been an inspiration for young dentists who aspire to become sound clinicians and want to confidently practice clinical endodontics. In Mission Smile Dr. Shruti heads the endodontic department which is one of the best in the city. Doing endodontics in real life situations is a completely different ball game and cannot be learnt from bookish courses. And thats the USP of our curriculum in Mission Smile where Dr. Shruti focuses on how to prepare the right access cavity, find the canals, clean and shape them and obturate them the right way keeping things simpler for a beginner. She has shaped many clinicians from start to finish who practice endodontics with ease today.
Dr. Angshuman also has integrated CBCT technology into his practice since last 10 years as he introduced Cone Beam CT to dentistry of Kolkata by starting Pioneer Scans (A Dental Imaging company) with its branches in Howrah, Lake Town and Golpark. He also introduced the first organised Medical Emergency Kit for Orthodontic and dental clinics in the name of Denlife which is being used pan India. He was recently felicitated in Align India Forum 2023 in Kochi in recognition for achieving Platinum status in India as a Invisalign Provider. Dr. Angshuman has lectured in Orthodontics in several platforms and has chaired many important sessions incuding international conferences. He has a net clinical experience of 23 years.
Clinical Concepts in Contemporary Modern Endodontics
- Biomimetic Principles: Prioritize conserving tooth structure using biologically compatible materials.
- Advanced Imaging (CBCT): Enables precise diagnosis and planning with 3D visualization.
- NiTi Rotary Instruments: Offer efficient and safe canal shaping with flexibility.
- Enhanced Irrigation Techniques: Sonic/ultrasonic methods improve disinfection and cleaning.
- Regenerative Endodontics: Revitalizes pulp in immature teeth through tissue engineering methods.
Rubber Dam Placement in Endodontics
- Isolation: Keeps the tooth dry by blocking saliva during treatment.
- Visibility: Improves access and view of the operating area.
- Infection Control: Lowers bacterial contamination of the canal.
- Patient Safety: Prevents ingestion or inhalation of instruments or debris.
- Better Outcomes: Increases success rates by ensuring a clean environment.
Access Opening After Anesthesia and Rubber Dam Application in Endodontics
- Purpose: Creates an entry point to reach the pulp chamber and root canals.
- Location: Made on the occlusal or buccal surface based on tooth type.
- Technique: Uses rotary or hand tools to remove structure while preserving dentin.
- Canal Identification: Enables clear visualization to locate canal openings.
- Treatment Preparation: Sets the stage for cleaning, shaping, and filling procedures.
BMP and Irrigation in Endodontics
- Biomechanical Preparation (BMP): Involves cleaning and shaping canals to eliminate infected tissue and prepare for filling.
- Irrigation Purpose: Flushes out debris, bacteria, and toxins to support disinfection during BMP.
- Common Irrigants: Sodium hypochlorite, chlorhexidine, and EDTA, each with distinct antimicrobial and cleansing properties.
- Irrigation Activation: Sonic or ultrasonic methods enhance irrigant reach and effectiveness.
- Safety Precautions: Correct technique prevents irrigant extrusion and protects surrounding tissues.
Performing Obturation in Endodontics
- Definition: Obturation seals the cleaned root canal space to block bacteria and promote healing of surrounding periapical tissues.
- Materials Used: Gutta-percha with sealers is commonly used due to its adaptability, biocompatibility, and sealing ability inside the canal.
- Techniques: Methods include cold lateral, warm vertical condensation, and carrier-based systems, chosen based on canal anatomy and clinical preference.
- Airtight Seal: A proper seal from the crown to apex prevents reinfection by eliminating spaces for bacterial leakage or fluid entry.
- Post-Obturation Care: Final restoration, like a crown, strengthens the tooth and protects the sealed canal for long-term success.
Crown Preparation and Impression Making in Endodontics
- Crown Preparation: Involves reducing the tooth after root canal therapy to accommodate a crown that restores strength and function.
- Tooth Structure Shaping: The tooth is shaped with smooth, tapered walls and a flat top for accurate crown fit and stability.
- Margins: Well-defined margins are crafted at the base to ensure a snug crown fit and protect the sealed root canal.
- Impression Making: Precise molds are taken using materials like silicone to record the tooth’s contours for custom crown fabrication.
- Temporary Crown: A provisional crown is placed to safeguard the tooth and maintain bite function until the permanent crown is placed.
Passive Crown Cementation in Endodontics
- Passive Fit: The crown is fabricated to fit passively over the prepared tooth, avoiding pressure that could harm tooth structure.
- Surface Preparation: Both tooth and crown surfaces are cleaned, dried, and sometimes etched or primed to enhance cement bonding.
- Cement Application: Cement is applied inside the crown to fill microgaps and bond it securely to the prepared tooth surface.
- Seating the Crown: The crown is gently positioned and seated using finger pressure or tapping tools for precise fit and alignment.
- Excess Cement Removal: After setting, excess cement is carefully removed to prevent soft tissue irritation and ensure a clean restoration.
Handling Endodontic Accidents in Private Practice
- Stay Calm and Assess: Evaluate the type and severity of the complication, such as instrument fracture, perforation, or material extrusion.
- Communicate with the Patient: Clearly explain the incident, expected outcomes, and proposed solutions to maintain trust and transparency.
- Use Specialized Tools: Employ endodontic retrieval systems or repair materials like MTA to manage procedural mishaps when appropriate.
- Referral to Specialist: For complex cases, refer promptly to an endodontist to ensure the patient receives expert-level care and attention.
- Document and Follow Up: Record the incident and actions taken, and schedule follow-ups to track healing and ensure proper resolution.
Managing Medical Emergencies in a Dental Practice
- Preparedness and Training: Ensure all team members are BLS-certified and trained in CPR, choking response, and AED usage.
- Emergency Kit and Equipment: Maintain a fully stocked kit with emergency drugs, oxygen, suction, and an accessible AED at all times.
- Recognize Early Signs: Watch for symptoms like fainting, allergic reactions, chest pain, or low blood sugar to act before escalation.
- Activate Emergency Protocols: Stabilize the patient, call 911, and administer appropriate care based on the practiced emergency response plan.
- Post-Emergency Steps: Document the event thoroughly, confirm patient stability, and schedule follow-up to assess recovery and complete records.
Practice Management in an Endodontic Course
- Business Operations: Teaches financial planning, staffing, inventory, and daily management to run a profitable and efficient endodontic clinic.
- Patient Communication: Builds skills to educate, reassure, and manage patient concerns with clarity, empathy, and professionalism.
- Marketing and Growth: Covers strategies like digital marketing, referrals, and outreach to attract new patients and retain existing ones.
- Legal and Ethical Considerations: Stresses the importance of informed consent, accurate documentation, and maintaining patient confidentiality and safety.
- Clinical Workflow Optimization: Offers methods to streamline scheduling, treatment planning, and follow-up for a smoother, patient-focused practice.
